For most tailors providing alterations is more lucrative as it’s higher volume even though less money per transaction. Making custom clothing is very time consuming with multiple fittings and because of the labor cost few people can afford the cost. There are many tailors in London, Paris and New York who still make bespoke clothing and even some larger men’s clothiers have bespoke lines (Hickey-Freeman, Samuelsohn, and others). Nordstrom’s provides bespoke tailoring for several men’s clothiers as do other very high end retailers. Those are probably the most accessible in the US.
